Ok, verified that this is indeed a ROM dump of a Lotus 123 cartridge for the PCJr. Yes, they actually made a cartridge of Lotus 123!
Here is the voodoo needed to make it work:
First, it currently only works in MESS version 148. Unsurprisingly MESS version 149 breaks it.
Second, to make this work you need an IBM PC-DOS 2.0 360k disk image. Other versions of DOS may or may not let this work. MESS will crash if you omit the disk.
Put the lotus123.zip (do not change the name) file in your ROMs folder, and put the PC-DOS 2.0 360k disk image in the software folder.
Then, at a command prompt, in the MESS folder, using MESS version 148, type the following:
mess ibmpcjr lotus123 -flop software\dos2.img
Note that you do not specify a path to the rom file. And here dos2.img is your PC-DOS 2.0 disk.
Finally, when the emulated PCJr is booted to a PC-DOS 2.0 command prompt, enter 123. Even though that is not a file on the disk, it will start the Lotus 123 application.

Don't bother downloading this file. The zip archive contains two uncompressed files lotus123.a and lotus123.b, which bizarrely are both 65,536 bytes long. They seem to contain bits from Lotus 123 revision 1A, but the files are not any kind of archive format, and they are too small to contain the entire thing. (123 1A was four 320k disks)
