Eclipse Europa:
1. Make sure that JAVA_HOME is set to your JDK 5 installation (If you don't have it, you can make a throwaway account on oracle's website to download it)
2. Once you've started Eclipse, select Help/Software Updates/Find and Install...
3. On the dialog that should have appeared, choose Search for new features to install and select Next
4. Select New Archived Site... and select the ADT archive you downloaded, then select Finish
5. In the dialog box that should appear afterwards, select Developer Tools and click Next
6. Accept the TOS, and click Next, after that, click Finish
7. Accept the installation by selecting Install All
8. Restart Eclipse, and your plugin is ready, now go develop some android apps!
Eclipse Ganymede (recommended):

2. Once you've started Eclipse, select Help/Software Updates...
3. On the dialog that should have appeared, select Archive... and click Add Site...
4. Select New Archived Site... and select the ADT archive you downloaded, then select Ok
5. Select the item that's name should start in jar:file: and end in ADT-0.8.0.zip and click Install...
6. In the resulting dialog box, make sure both Android Development Tools and Android Editors are selected and click Next
7. Accept the TOS, and click Finish
8. After the installation is done, Restart Eclipse and your plugin is ready, now go develop some android apps!
